Quick and short update:

Fell into the trap in the Nightmare Frontier, but overall made nice progress.
Opened the shortcut in the ghost infested castle, still some stuff to explore there.
Killed the White Woman of Byrgenwerth and fought Rom. Almost had him but a spider got me while finishing him off. Not sure if the choice to ignore them was the best, but it almost worked.
Back to grinding for some ressources, since I blew a lot of bullets/potions.

I found the cannon, but don't have the stats to use it. (may have them now though)

I killed the Darkbeast & Rom and am currently fighting some idiotic strong hunter in the cathedral.
While he does most of the dmg to himself, due to his weapon, I didn't manage to kill him yet.
The wooden shield helps a lot though, since his gun hits like a truck.
...
Now that I remember, forgot to change my armor after Rom (magic res. focus). That may be the reason he is so bloody dangerous.

Otherwise, the moon is now red. Noticed a new enemy while grinding for stuff at the Central Yharnam lamp, which seems to be the guy from the window (dropped a rune). Also, the Bagmen seem to be gone.
Also worth of note, in Byrgenwerth was a note indicating the birth of an elder god or something and suddenly the prostitute in the church seems to have tummy ache.

Otherwise, the Unseen Village has been seen.

Found the Upper Cathedral Ward though I died before I could find a bonfire there. Ran out of resources again... back to the old grindmill.
Also found a Tiny Tonitrus, which I can't quite use... yet :-D

Weird little thing while grinding for a bit.
Went back to repair my weapons and there was a loud fire noise in the house. Equally distributed, not from the fireplace. Left the house, the noise vanished. Re-entered, the noise was still gone. Some Insight thing? (currently at 70)

Not so far. Just grinded for a bit, upgraded my stats and weapons. Unlocked the church bonfire in the unseen village and killed the dude in the cathedral, yielding the crow ladies stuff.
Also ran into the Amygdala, but didn't kill her yet.

I had a troubled childhood. I took the cane and (I think) the pistol. Character creation and the opening/hub areas are nowhere near as engaging as they were in Demon's Souls and Dark Souls unfortunately, but it definitely has all the hallmarks of those predecessors. I'm not a big fan of the gangly appearance of my dumb character. I didn't get far. I'll continue apace.

I picked up the Add-on and quickly jumped into it for a few minutes:

To get there, you get two items in the dream and head outside of the chapel and be killed by 'You know what'. While I was there, I realized the little old lady was gone and the guy on the floor said that she was killed outside. Found her corpse and I guess I need a new sedative supplier.
Upon death an unfamilliar voice spoke and I woke up in a nightmare version of the cathedral district. After fighting a few nasty hunters, who seem to be the respawning standard enemies here (some wielding new weapons) I found my way to the entrance of Amelia's old place, though it was guarded by two hunters so I took off elsewhere, and the gate (like in the real world). It's worthy mentioning that the hunters activly engage with the local beasts. At some poin, as I turned around a corner I ran into 3 beasts and attacked them in surprise, just to notice that they backed away scared and didn't even fight. I feel a bit bad now... The only loot so far is a set of Old Hunters garb and consumables/crafting stuff.

Sidenote: I now hear a baby cry at Odeons Chapel and strange moaning noises in the Hunter's Dream. Insight?
